The OVA version of To Love Ru (which I don't believe was released in the US) has a number of DID scenes as comic relief:
Episode #1 (around 7:30) - One of Lala's inventions malfunctions and turns Rito into a girl. An option to turn her back to him is surgery, so we see a glimpse of her/him clamped to an operating table.
Episode #4 (eyecatch) - A quick shot of Lala in chains. Then, around 14:00, Lala is tied with thorny vines.
Episode #5 (around 15:00) - Yami has gotten drunk on alien pollen and wraps her weaponized hair around some of the other girls. Then, around 18:00, the vines of an alien plant do the same (though restraining their arms, too).
NSFW warning: the amount of fanservice in the OVA is much higher than in the TV series.
