I had seen vidcaps of the DID scenes in episodes #14 and #16, but I didn't know Guilty Crown had other DID (and GID) scenes as well until I watched the show:
Episode #1 - girl blindfolded and handcuffed in front
Episode #2 - several men blindfolded, tape gagged, and with their arms behind (we don't see the bindings)
Episode #3 - dude hanging upside down; the lighting is poor but it appears that he's in a straightjacket with both a blindfold and an OTM gag
Episode #4 - dude handcuffed behind
Episode #7 - two men tape gagged (I'm suddenly having a hard time remembering the scene, but I don't think we can see the bindings)
Episode #11 - old guy blindfolded and shackled to a solid chair of sorts
Episode #18 - girl entangled in a cable
Episode #19 - (1) girl with arms chained overhead, (2) dude with arms behind him on a leash to the wrists (but the camera angles make it hard to see exactly what the bindings are), and (3) a girl trapped in a crystalline substance with her arms overhead (continued into the following two episodes)

I could never tell if Ayase's second gag was cloth or tape. Anyway, someone of staff must have had a fetish or something.

I think it tape, because because we don't see the gag-knot. Also for me at least it seem that the gag is more round at the upper part, and has a straight line in the lower part. What is still a question for me is why would she be tied up with rope, and then over taped (because of the line between the two duck-tape). So it's a hard one to really decide
