Thought of a few more to add to this list:

Fantaghiro ep 24 - The little fairy princess girl is tied and OTM gagged when she's thrown out of that floating pot thing by the baddies who captured her, but as she's falling and caught by Fantaghiro's boyfriend guy, the ropes disappear! The next time we see her on a leaf with that guy, her gag is gone too. But when it cuts back to them again a few minutes later, she's both bound and gagged again like the previous scenes never happened and the guy is untying her gag!

Police Academy ep 22 - When the bad gang kidnaps that hotel manager lady and is leaving the place on their surfboards, she's bound and OTN gagged as they speed toward the front door. But as they're shown leaving the building, she's both completely untied and ungagged! Would they really bother to free her as they make their escape!? When we next see her again she's tied and OTN gagged in the speedboat. Her OTN also changes to an OTM in the final shots, but this can happen, so no biggie.

Police Academy ep 47 - Watch this scene again and you'll notice that Debbie Callahan's gag changes back and forth between OTM and OTN a few times, and also makes a color switch! In the first few scenes where she's gagged, it's a light beige sort of color, while in the latter half it's plain old white.

I came across another good example to revisit this topic.

It's from Bosco Adventure episode 17.

Image
First we have Apricot with hands tied behind the back of the chair.

Image
Next we have Apricot with hands behind her back with more ropes around chair and across her lap.

Image
Now ropes across her lap has disappeared.

Image
Her hands are I assume are now back around behind the chair with chest ropes now around her torso instead.

Image
Finally back to ropes around chair but the lap ropes are still missing.

All this from the same scene beginning to end.

Another example of "Now you see it now you don't".

and I thought the only errors I've seen in animations is that some shows would have the voices mix up. You know the characters voice would come out of the wrong persons mouth.

The ones that turn me off is when the thumbs are drawn wrongly during hands-behind-back angles.
